First register on e-Vidhyut Seva portal.
After registering login with Mobile Number and OTP.
After that Link Your Customer Number in portal.
Click on LT Name Change from the home page.
Then enter the New Customer Details (Customer Name, etc).
Then Upload the required documents.
Now make the Payment and Download your application form.
Now your application will go to corporate office in CPC where your uploaded documents will be checked. Then your application will proceed.

Registration on Portal

For Name Change Application first of all if you are not registered with e-Vidhyut Seva then register. Go to e-Vidhyut Seva Seva portal from the link given below.

If you go to the above link, the home page of e-Vidhyut Seva portal will open as below, from which you have to click on Signup link given below.

Now the below page will open in which you have to select your company DGVCL and enter your Mobile Number, Password for 2 time and Email and click on Sign up button then your registration will be done.

After registering as above you have to Login to portal which you can do by following below steps.

First of all go to the home page of e-Vidhyut Seva portal which can be go from the link given below.

Now the below given page will open in it enter your Mobile Number and select company DGVCL and click on Login button then enter OTP to login.

Add or Link Your DGVCL Consumer Number

After login now you have to add your DGVCL Customer Number. Here you can add more than one customer number. You can add as many customer numbers as you want.

After login, the below page will open from which you have to click on Manage Account.

Now click on Add LT Account button from below page to add Customer Number in portal.

Now the below page will open in which you have to enter your Consumer No, Bill Date, Past Reading from your bill.

For above information like Bill Date and Past Reading you have to check your bill. If you don’t have the bill then you can check your Bill Details from the link given below and enter the details as given in the below photo.

DGVCL Name Transfer or Change Application

After adding the customer number as above go to the home page and click on LT Name Change given on the left side which is shown in the below photo.

Now as below the page will show as many customer numbers as you have added in the drop-down menu. Select the customer number in which you want to change the name and click on the Submit button given below.

Enter the Details of the New Customer

Now as below page it will show old customer details like Old Customer Name, Category, Load etc.
Now in the below page you have to fill the details of the New Customer. And how to fill the following details is given below.

Name : Enter New Customer Name
Reason : Select the reason of Name Change.
Security Deposit : In which two options are given.

Entire amount to be paid by consumer
Difference amount to be paid by consumer

Security Deposit, if you have buy house, then select first option of new deposit, because if the old customer wants his deposit return then he can apply for it and get it back.
Security Deposit, if the house is inherited (Varsai), then the second option is to select the different amount by the new customer, because amount has to be adjusted with the old deposit and you just need to pay different amount.

Old Security Deposit Amount Already Paid by Consumer : This will show the old security deposit amount already paid.

After entering the information as above, click on the Submit button.

Upload Documents

Now the below page will open in which you have to upload your documents.

Photo Graph : Here you need to upload your passport size photo. Photo size should be under 20kb and in .jpg format.
Signature : Here you need to upload your signature. Its size should be under 20kb and in .jpg format.
Identity Proof : In this you need to upload one from Aadhaar Card, Election Card, PAN Card etc. Document format should be .pdf.
Ownership Proof : In this you need to upload the proof showing the right of ownership of the premises. Like Akarani Letter, Tax Voucher (Vera ponch), Municipal tax receipt, Dastavej copy etc.
Original receipt for payment of Security deposit : In this upload your old deposit is receipt if you have if don’t then leave it.
Consent of partner on Rs. 300 Stamp (Duly notarized) if property is jointly owned : If the property is jointly owned more than one name, then notarized consent letter of all other member in 300 stamp paper.

After uploading the above documents, click on the Submit button given below.

A1 Form Download

Now the below page will show how much rupees you have to pay and the link for payment will be given. Your Application Number and A-One form download link will appear. Click on View Your Application (A1 Form) to download your form.

Now your A-One form will open in a new tab as below, click on the Download button given above to download it, your A-One form will be downloaded.

Now after downloading the form, click on the Pay Now button from the below page to make the payment.

Make Payment

Now read some conditions given in the page as below and tick the check box and click on Submit button.

Now in the below page you need to enter your Application Number and captcha code and click on Submit button.

Now in below page you will see Customer Name and Payable Amount as below and click on Pay Now button given below.

Now the Disclaimer page will appear, there will be a Continue button below, click on it and then the dialog for payment will appear as below. From there you can make your payment by selecting any payment method.

Now click on UPI and scan the following QR Code from any of your payment apps and make the payment.

Now after your payment is done, your Payment Receipt will open as below and click on the Print button given below to save it, the PDF will be saved.

After payment your application will go to CPC at Corporate Office. CPC will check your uploaded documents and may accept or reject if any query (if the documents are not suitable).
CPC may reject the document with proper reason if the documents provided by the applicant are not correct. The applicant has to re-upload the same document on the portal.
In this way, your name change application will be done, the status of which can be checked by going to Check Application Status.
